Application for Enrolment

We suggest you print the forms below, fill them in as thoroughly as possible and fax them back to Youthlink Family Trust on 09 236 9565. Once we receive your application you will be contacted by our Liaison and Referrals Manager.

When we have the information requested we will be able to provide an approximate timeline for meeting with the young person, their family/whanau, Social Worker and all relevant parties to assess the childs needs and, as appropriate, create an individual care plan and opportunity for admission.
